Fibaro FGS222 turns off immediatley

I've got a Fibaro DOUBLE RELAY SWITCH FGS-222 installed with a rotary selector switch (so basically 2 input switches which are mutually exclusive) to control my home's ventilation system (all off low, switch 1 medium, switch 2 high). I've tried various things, but no matter what, when I activate input S1, the output Q1 turns on for about 1 sec and then turns off again. Same when turning on via Domoticz switches and the ozwcp page. Using S2 to switch Q2 works fine, same via Domoticz switch and via LUA.

So the questions is: do you have any hints how to fix this?

I tried:
parameter 1 all on/off both enabled and disabled
parameter 2 manual override on and off
parameter 4 auto off delay is set to 0 (which should disable auto off)
parameter 13 input behavior I tried both 'toggle' and 'follow switch'
parameter 14 set to bi-stable

And nothing seems to work.
