Device's getting forgotton about / dropping off

Last few weeks, i've had issues with 2 devices jsut getting bored and stop communicating.
I've tried all sorts to node heal, refreshing node info, excluding and including it and none of this worked.

I managed to eventually include the node, but now i have 2 of the same device that used to be the device, but will not remove from my openzwave list.

How do i get rid of them? I tried the delete button in domoticz on the z-wave device list.
It disappears off from that list, but not the openzwave list.

Also, i've also had issues with devices just getting forgotton about altogether.

For instance, a window sensor dropped off all lists.

What is causing all these issues?
I'm running pi2 with a Razberry 1

Now setup on a RPI3.
Also using Evohome HGI80, RFXTRX433E and a Aeotec Gen5 stick. Mainly a Gen5 Z-Wave system.
My weather (Davis vantage Pro2) data is obtained by weatherunderground
Now collecting rtl_433 data from Apollo Ultrasonic meter

ah excellent, going through the editing the .xml file has finally forced those devices are dead.

Just stuck there, but i'll get there now they're marked dead.
